If you plan to travel, please complete our online pre travel assessment form (opposite) at least 6 weeks before your travel date and give it to the Practice. Ensure the surgery has a current contact phone number encase any details need to be clarified with you.
Please allow 3 working days before calling back. The practice pharmacist will leave a letter with travel information and if required an NHS script. If a script has been attached please book a treatment room appointment for administration.
The letter will also indicate recommended/optional vaccinations which are private (not covered on NHS). The surgery does not provide private travel clinic services, so if you feel you need any of these please attend a Private Travel Clinic.
